Reporting an Incident or Concern

At ÅÝܽ¶ÌÊÓƵ, the health, safety, and well-being of our community are our top priorities. We encourage all faculty, staff, and students to report concerns to help us maintain a safe and supportive environment.

Below you'll find links to report a variety of issues. Please use the appropriate form to submit your report.

  • Sexual Misconduct: Report incidents of sexual harassment, discrimination, or assault.
  • Student Misconduct & Concerning Behavior: Report violations of the Student Honor Code or share concerns about a student’s personal well-being or behavior that may pose a risk.
  • Academic Dishonesty and Misconduct: Report any suspected violations of the Academic Honor Code, such as cheating or plagiarism.
  • Student Grievances and Complaints: Submit formal complaints or grievances if you believe you have been subjected to an alleged inequality in violation of university policies and procedures.

General Concerns: Report other issues or incidents that affect the university community

Reporting Sexual Misconduct, Discrimination, and Retaliation

If you have experienced sexual harassment, discrimination, retaliation, or any other harm based on sex, you can use this form to file a report.

While we encourage you to use this form, you also have the option to contact a university official directly:

All reports are handled with confidentiality in accordance with FERPA and university policy.

Student Concerns and Incident Reporting

If you are concerned about a student's well-being or have observed a violation of the Student Honor Code, please use the Student Concern & Incident Form. This form can be used for a variety of issues, including:

  • Behavioral Concerns: Referring a student to the Health and Wellness Student Services Team due to concerns about their personal well-being or potential to harm themselves or others.
  • Code of Conduct Violations: Reporting a suspected violation of the Student Honor Code.

For immediate danger, please call 386-481-2900. If you are using the form for an urgent matter, be sure to mark it as "urgent."

Academic Dishonesty and Misconduct

Use the Academic Dishonesty and Misconduct Form to report any suspected violations of the Academic Honor Code. This includes, but is not limited to:

  • Cheating
  • Plagiarism
  • Dishonesty with course materials
  • Theft or damage of intellectual property
  • Alteration of university documents
  • Classroom disturbances

Student Grievances and Complaints

If you believe you have been treated unfairly or that a university policy has been violated, you can use the Student Grievance & Complaint Form. This procedure allows students to present an issue that they feel warrants action, including the right to secure educational benefits and services.
